WHAT TO EXPECT

  • The submission form will ask for all contact information +basic PA info

  • I will contact you via text or email to let you know it has been received

  • I will clarify any questions I may have in that initial contact

  • Please follow the link at the end of the form to submit your docs OR

  • Please provide the Docusign/Authentisign log-in so I can access docs
